Chemong Family Health Organization

The MMC is fortunate to be able to provide primary healthcare services to our community. The team of physicians at MMC focuses on health promotion, disease prevention and chronic disease management. As part of the Chemong Family Health Organization, the physicians work with allied healthcare  professionals to provide exceptional care to their patients. The Chemong Family Health Organization is affiliated with the Peterborough Family Health Team or ‘PFHT’.

Please note that the doctors at the MMC work with ‘rostered’ patients, who have been registered with them, generally through the Health Care Connect Service. The allied, healthcare professionals, such as dietitians, mental health professionals and nurses who work at the clinic can be accessed only through referral from one of the doctors. These professionals do not make appointments directly with community members, but work as a team with one or more of the doctors to provide excellence-based, holistic primary health care.

Extended Hours Service
Patients connected with a family doctor at the Morton Medical Center have access to the Extended Hours Clinic.

Extended Hours clinic is available for rostered patients during the following hours:

Saturday 9:00 AM to 3:30 PM

The Administrator of the Chemong Family Health Team, Jessica Hope, is responsible for the overall management of the team and its resources. The Administrator provides support to the physicians while ensuring all activities are compliant with the Ministry of Health.

The goal of our care team is to provide balanced healthcare management for the whole person. We aim for continual improvement in the way we deliver care. Together, we are happiest when our patients achieve good health outcomes.

Our team includes Dr. Arthur, and an Advanced Practice Registered Nurse who conducts women’s health reviews, provides counselling to encourage good health habits, does wound care, and, along with Dr. Arthur, supports those who are living with diabetes. An additional RN meets with patients to help them manage blood pressure, cholesterol levels and other health risks. The office staff also includes a receptionist and administrative assistant.

Dr. Jeffrey Boyes earned his medical degree from Queen’s University in 2003, and completed his residency in family medicine at McMaster University in 2005. Following graduation, he continued to practice in Hamilton at a community health centre and an urgent care clinic. He joined the team at the Morton Clinic in April of 2015, assuming the care of patients previously rostered to Dr. Chris Bailey, upon Dr. Bailey’s retirement. In 2016 he earned the designation of Fellow of the College of Family Physicians of Canada. Jeff’s partner Jennifer works alongside him as a Registered Practical Nurse, and has a special interest in well woman care and foot care.

Dr. Boyes’ receptionist can be reached during office hours at 705 652 6405. Office hours are currently 9am-4pm weekdays, closed on Wednesdays. A voice message, with instructions on how to contact the physician on call, will be provided outside of business hours and when the office is closed for holidays. On some Tuesdays, an evening clinic will replace morning office hours. A Saturday clinic will replace the Friday clinic once every few months.

Appointments: Both in-person and phone appointments are available, depending on the patient’s preference. Our goal is to offer appointments on the same or the next business day, however, if a patient feels that the appointment offered is not soon enough, a message can be left for Dr. Boyes to review the matter and will be answered the same day. Therefore, rostered patients are asked to make every effort to be seen at the Morton Clinic for all non-emergent issues. Appointments are ten minutes in length. Patients will be asked to list all symptoms at the beginning of the appointment so that they can be prioritized, as further appointments may be needed to address all of them. A current health card must be brought to each in-person visit. Patients who do not provide 24 hours notice prior to cancelling an appointment may be billed directly for it.

Prescription Renewals: The renewal of prescriptions outside of scheduled appointments is not covered by OHIP, and a fee will be billed to the patient when our office receives and acts on faxed renewal requests from pharmacies. Patients will have the option to pay a yearly block fee that covers this and other uninsured services. Patients not wishing to pay for faxed prescription renewals will need to schedule an appointment to renew their medications, and instruct their pharmacy to not fax renewal requests.

Test Results: Results of all tests and investigations will be communicated to the patient by Dr. Boyes, along with his recommendations, through a phone call from his receptionist or nurses. If there are further questions or concerns, the patient will be booked with Dr. Boyes to discuss the results and recommendations in more detail.

Dr. Dan Houpt graduated from McMaster Medical School in 1993. A former high school teacher, he has been practicing medicine since 1995. For six years he was a Family Physician at the Peterborough Clinic and from 2001 until 2013, he has been working as an Emergency Room Physician at Peterborough Regional Health Centre. He resumed his Family Medicine practice at the Morton Community Health Centre in September 2013 following the very unfortunate death of Dr. Steve Senior.

Dr. Houpt lives on a farm in Douro with his beautiful wife Kelli, five children and
any number of horses, barn cats and a goat. He enjoys gardening, fishing and bike riding and plays a mean guitar in the Indian River Band.

Dr. Hughes has been practicing medicine since 1999. Dr. Hughes obtained his medical degree at the Memorial University of Newfoundland, completing the Family Practice residency program at McMaster University. Dr. Hughes has an active family practice. He is a member of the Canadian College of Family Physicians and the Ontario Medical Association.

He started his medical career as part of the Canadian Forces and spent several years working as a hospitalist in both Calgary and Peterborough before starting his current family practice. Dr. Hughes has been a part of the Canadian Red Cross, serving as the ‘Medical Director of First Aid Services’ for the Ontario Zone and as a member of the National Medical Advisory Committee since 2000.

Dr. Hughes has also been involved in various aspects of medical administration during his career, including Base Surgeon Duties for Canadian Forces Base Borden and the Senior Medical Officer for Royal Canadian Regiment’s Battle Group for the Stabilization Force for Bosnia-Herzegovina in 2001. In addition, he has been Deputy Lead for the Hospitalist Program and the Vice President of the Medical Staff at the Peter Lougheed Centre in Calgary.

Dr. Hughes enjoys a variety of activities outside of his profession including traveling, participating in a variety of sporting activities, local coaching and sports training, and spending time with his family.

Dr. David HURST, M.D.

Dr. Hurst was a family doctor for many years in Peterborough. In 2007 he accepted an offer from the Queen’s University School of Medicine to become a Professor and teach medical students in Kingston. For the next ten years he looked after a large family practice as part of the Kingston Family Health Team, at the Centennial Family Physicians Clinic.

When a position became available at the Morton Community Healthcare Centre in 2018, he and his wife Kathleen decided this would be a great opportunity to move back and live on a farm closer to their children and grandchildren.

The office team includes Dr. Hurst and two Registered Practical Nurses (RPN’s). Our goal is to provide excellent health care to all patients.  Our nurses have additional training and expertise in Diabetes, Women’s Health and Well Baby Care.  We are striving to provide the best medical care in these times, with a balance of booked “telephone virtual visits” and booked “in person” appointments.

The Extended Hours Clinic is for patients of the Morton Community Healthcare Centre on Monday to Thursday (5-8pm) and Saturdays (9am-3:30pm).

Dr. Munoz

Background
Dr. Munoz was born in Kelowna B.C. He attended the University of Washington and then went on to McGill University in Montreal to complete his Bachelor of Science. He also obtained his Medical Degree and completed his residency at McGill University. Dr. Munoz practiced in Bancroft for 18 years prior to moving to Lakefield with his family. Dr. Munoz also currently works in Emergency Medicine at both the Peterborough Regional Hospital and Quinte Health Care, North Hastings Hospital in Bancroft. Dr. Munoz enjoys spending time with his wife and four children on their farm.

Preparing For Your Visit
It is very important that you bring your Health Card to each appointment. Also, please bring your medications or list of current medications with dose and frequency. Check your prescriptions before coming so you will know what needs to be repeated as we charge for medication renewals by phone or fax. In general, one primary health complaint will be addressed per visit.

Missed Appointment Policy
We understand that there are times when you can’t make an appointment due to an emergency or a work or family obligation. In these situations, please cancel your appointment by phone 24 hours before your scheduled appointment. Missed appointments that are not cancelled ahead of time prevent other patients from accessing appointments. Additionally, last minute cancellations can cause problems and added expenses for the office. For these reasons, the office has a 24 hours cancellation policy or there will be a fee.

Uninsured Services
Not all services provided are covered by OHIP. Some examples of these uninsured services include sick notes, medication renewals by phone and the completion of certain forms. We charge a $15.00 fee to renew medications over the phone. All uninsured services are billed based on fees suggested by the Ontario Medical Association.

Coverage
Dr. Munoz is in the office on Mondays and Thursdays. He has an evening clinic approximately every eighth Friday. He also shares the office with Dr. Wilson who is in the office on Tuesdays, Wednesdays and some Fridays. Dr. Munoz and Dr. Wilson will see each others’ patients for urgent, same-day appointments. After hours access is available to Dr. Munoz’ patients from Monday to Friday evening from 5-8 pm for urgent cases.

Background

Dr. Majeed graduated from the University of Toronto School of Medicine in 2010. She completed her Family Medicine Residency at the University of Toronto as well. She has a special interest in medical aesthetics but otherwise enjoys every aspect of Family Medicine. She really enjoys the diversity that family practice brings- seeing patients of all ages and backgrounds – and working alongside her amazingly skilled and dedicated staff.

Dietitian

Dietitians provide patients with one-on-one counselling, as well as group sessions regarding nutritional values that will assist in maintaining a healthy lifestyle. Dietitians provide advice on how nutrition and diet impact your health, as well as providing disease prevention and treatment services for diabetes, heart disease, and other conditions. Our dietitians are registered with the Dietitians of Canada, and the College of Dietitians of Ontario.

The dietitian accepts referrals from your physician and then can collaborate on your healthcare needs. Members of the community cannot make appointments directly with the dietitians. This service is available to rostered (i.e. registered) patients of the centre.

Pharmacist

Pharmacists provide medication-related education and support to patients with complex illnesses such as heart disease and diabetes. The pharmacist can also do a medication review with patients to assist with medication compliance and any questions or concerns with current medications.

The pharmacist accepts referrals from your physician and then can collaborate on your health care needs. Members of the community cannot make appointments directly, or have their prescriptions renewed, by the pharmacist. This professional provides services to patients registered with doctors at the MCHC.

Mental Health Clinician

Mental Health Clinicians help resolve problems by providing counselling and therapy and by finding required community resources where and when needed.

The mental health clinician accepts referrals from the physician with whom you are registered and then can collaborate on your health care needs.
